Cancer Risk Triples in Kids with Arthritis (CME/CE)

Children with juvenile idiopathic arthritis (JIA) had a nearly threefold increased risk for malignancy, even if they had never been treated with biologic therapies, researchers found.

The overall incidence rate of cancer among children with JIA who had received only conventional therapy was 67 cases per 100,000 person-years (95% CI 1.3 to 132.5), while the rate among healthy children was 23.2 cases per 100,000 person-years (95% CI 12.2 to 34.2), according to Beth Nordstrom, PhD, of United BioSource Corporation in Lexington, Mass., and colleagues.

Accordingly, the hazard ratio for cancer among biologic-naive children was 2.81 (95% CI 0.94 to 8.34), the researchers reported in the September Arthritis Care & Research.

Adults with rheumatoid arthritis are at increased risk for certain cancers, particularly lymphoma, which is thought to relate to the underlying immune system dysregulation and high levels of systemic inflammation.

In addition, treatment with tumor necrosis factor (TNF) inhibitors may contribute to the risk, because of the role TNF itself plays in tumor surveillance.

Since children with JIA began receiving treatment with the biologic agents, questions have arisen regarding cancer risks, adding to uncertainty as to whether the disease itself in younger patients also is associated with malignancy.

In an attempt to sort through the possible risks of the disease and its treatment in young patients, Nordstrom and colleagues conducted a case-control study that included 3,605 patients diagnosed with JIA before age 16 and 37,689 controls from a large administrative database.

Mean age was 11, and the majority were female.

During 5,974 person-years of follow-up, malignancies other than nonmelanoma skin cancer and carcinoma in situ were diagnosed in 0.1% of the JIA group.

In contrast, during 73,395 person-years of follow-up, 0.05% of controls were diagnosed with cancer.

The standardized incidence rates for both case and control groups were higher than the standardized incidence rates in the Surveillance, Epidemiology, and End Results (SEER) database, though only among cases was the rate significantly higher (SIR 4.03, 95% CI 2.56 to 5.99).

Patients ages 12 to 17 were at decreased risk compared to patients age 18 and over (HR 0.18, 95% CI 0.05 to 0.68).

The total numbers of probable or highly probable cancers were small, with four among JIA cases and 17 among controls, so separate analyses could not be done for most individual types of malignancies.

However, the SIR for lymphoma was significantly higher at 14.81 (95% CI 7.62 to 25.67) in the JIA group, "suggesting that lymphoma may be among the primary drivers of any risk of cancer in JIA," the researchers observed.

Two of the JIA patients had been treated with methotrexate, and calculation of their risk separately found an HR of 5.47 (95% CI 1.26 to 23.70, P<0.05), which was significantly higher than for all JIA combined.

A possible explanation for this finding was that methotrexate was likely to have been given to children with more severe disease, although an earlier study examining cancer rates before methotrexate became available in the 1980s did not rule out the possibility that there may be a methotrexate-related increase in risk.

Nordstrom and colleagues acknowledged that their study was not designed to assess risks associated with methotrexate, but rather those from treatment with biologics, which generally are only used when methotrexate fails.

"Therefore, it is reasonable to assess the cancer risk in methotrexate-treated JIA patients as a baseline for understanding any excess risk conferred by an anti-TNF agent," they argued.

Limitations of the study include its reliance on administrative claims data, a lack of information on race, and probable closer monitoring of children with JIA that could detect cancers more quickly.

And despite the large study population, the small numbers of JIA patients and malignancies overall do not permit definitive assumptions from being drawn about safety of TNF inhibitors in children.

"We propose that the interpretation of the risk of cancer in JIA patients treated with biologic agents should be made with consideration for the potential increased risk associated with JIA and conventional treatment," Nordstrom and colleagues stated.

Gone Series RP

an idea that literally just hit me:

there is a series of (unfinished) books called the gone series that is set in modern times and in a fictional californian town. a dome appears over this place and everyone above the age of 15 poofs out of the area, and reappears outside of it, leaving just the little kids to fend for themselves.

these kids slowly mutate and develop superpowers, of which range similar to a cell phone signal (between 1 and 4 bars, but there's only 2 known 4 bars the other is semi-dead), and they have no internet or phone service to connect to the outside world, they even have a limited food source. there eventually gets all sorts of in fighting.

so here's my idea: we RP in this series, but it would be loosely designed on the series itself, keeping just the dome, the powers, the threat of poofing on your 15th birthday, and the struggle of everyday survival.

we will RP as kids ranging from at least 9 to a maximum of 14, and you can be powered or not, or even get your powers later down the road (happens in the books as well lol)

Boeing tests CST-100 parachute protector

ScienceDaily (Aug. 29, 2012) ? The Boeing Company recently completed a jettison test of its forward heat shield, which will protect the parachutes of the company's CST-100 spacecraft during future missions to and from low Earth orbit. The forward heat shield jettison will start the parachute deployment sequence and provide a safe landing for the capsule and its crew members. The test was part of Boeing's work supporting its funded Space Act Agreement (SAA) with NASA's Commercial Crew Program (CCP) during Commercial Crew Development Round 2 (CCDev2).

"Without the parachutes, the crew wouldn't survive landing. We need to slow them down to a safe landing velocity," said Mike Burghardt, director of Spacecraft Development for Boeing's CST-100. "It is key to be able to get that forward heat shield released at the right time so we can deploy the parachutes."

Testing of the composite-based forward heat shield took place at Bigelow Aerospace's headquarters outside of Las Vegas on June 5, 7 and 11. Surrounded by dry mountain air, Burghardt and his team set up a series of high-speed and high-definition cameras to capture the shield's deployment, a sequence that begins with four piston-like thrusters firing at an altitude of between 20,000 and 30,000 feet. Once the thrusters fire, they are designed to push the shield out of the way of the spacecraft, allowing the drogue parachute to deploy to stabilize the descending crew module followed by the three main parachutes.

During the test, accelerometers and strain gauges measured the shocks and loads that were transmitted during the deployment sequence. The team also tested the ability to jettison the forward heat shield if one of the thrusters were to fail. The thrusters connect to the forward heat shield using breakaway joints, which are designed to fail if one of the thrusters did not fire during re-entry.

"The joint is designed so it won't break when exposed to nominal loads. However, if we did have a thruster failure case, the joint would break to allow the heat shield to be jettisoned from the spacecraft," Burghardt said.

The shield is an adaptation of what Boeing used for its Apollo moon capsules and is similar to the design NASA's Orion spacecraft will use for human exploration missions to deep space.

In March 2011, the agency signed a $92.3 million SAA with the company for the continued development of its Crew Space Transportation (CST), which is a reusable, capsule-shaped spacecraft designed to carry up to seven people, or a combination of people and cargo to low Earth orbit. Optional milestones valued at $20.6 million also were approved and will be completed later this year.

Boeing also tested the CST-100 service module propellant tanks on July 2 to make sure they can handle the extreme requirements of a launch abort scenario.

"The earlier you can do testing and the more testing you do accelerates what you learn," Burghardt said. "There's nothing like building something and then going to test it to find something you didn't think you knew about or you didn't remember."

Boeing will have the opportunity to conduct more testing on its CST-100 during NASA's Commercial Crew Integrated Capability (CCiCap) phase, which began earlier this month with three American companies that are advancing integrated spacecraft and launch vehicle designs. Boeing partnered with United Launch Alliance to integrate its capsule with an Atlas V rocket for launches to complete its integrated Commercial Crew Transportation System. CCiCap will set the stage for a crewed orbital demonstration mission in 2016, the start of operations that will lead to providing commercial transportation services to the agency to send crews to the International Space Station.

GOP convention not boom for Tampa merchants

TAMPA, Fla. (AP) ? Streets in the heart of downtown are eerily quiet, key intersections blocked off. Even the best restaurants are easy to get into without a reservation or much of a wait.

It's supposed to be an honor to be picked as the host city of a presidential nominating convention.

But so far, a combination of nasty weather, tight security that blocks off streets and a delay in Republican National Convention activities has left Tampa's streets deserted, its restaurants nearly empty. Some merchants remain hopeful they'll make up for it in the next several days, while others worry they've lost so much money from investing in extra staff, food and merchandise that no amount of business will put them in the black this week.

Jeff Morzella had hoped the convention would double business, but on Monday, only 75 customers ate in his restaurant compared to 400 patrons on a typical day.

"This has been a ghost town," Morzella said Tuesday morning, standing outside his restaurant named FRESH. Streets surrounding the block were barricaded. The biggest source of downtown traffic for the past few days has been police officers on bicycles, but they have been eating at meal stations catered by outsiders, not local restaurants, Morzella said.

FRESH generally garners up to $20,000 in weekly revenue but as of Tuesday had only taken in $800.

"More money out of pocket. No money coming in," said Morzella, whose restaurant serves soups, salads and paninis. It's on a row of restaurants just a few blocks from the Tampa Bay Times Forum where delegates are convening. "I would need to triple business between now and the end of the convention to make up for what I've lost already."

Tampa Mayor Bob Buckhorn said by the end of the week, the economic gains will outweigh losses, thought he acknowledged that downtown was "dead" Monday. The day was soggy with intermittent downpours from Isaac and downtown lacked many delegates since the Republicans delayed their activities for a day because of the storm.

Still, the Democratic mayor said the convention was expected to infuse $150 million to $175 million into the local economy with delegates spending about a third of that and the host committee spending another third.

"I think when we're all said and done with this, this will have a huge economic impact on the city," Buckhorn said.

And not all businesses are a bust. All area hotels are booked at a time when Florida's summer heat usually leaves them begging for guests. Limo drivers are in demand.

At Encore Select, which has taken over a storefront blocks from the Forum to sell "NoBama!" T-shirts and Mitt Romney buttons, revenue has only been half of what company president Richard Moore anticipated. Almost all patrons have been locals, not out-of-town delegates. Moore had hoped delegates would go shopping on Monday since there was little convention activity. If they did, it wasn't at his store.

"I don't know what they did yesterday. They didn't have anything else going on," Moore said Tuesday. "No one else came in. We would have been better off just having our local business."

And even in the area where many of the Republican delegates are staying, business was hardly booming.

Business at Ferg's Sports Bar & Grill in St. Petersburg was hurt by barricades erected around nearby Tropicana Field where some convention events were held. The lunchtime crowd of up to 200 patrons dwindled to about 75 on Monday, and owner Mark Ferguson estimated he had lost more than $10,000 during the last six days.

"The convention didn't help us at all," Ferguson said.

At the Jimmy John's sandwich shop in downtown Tampa, business was down from the typical $3,000 a day to $2,500 a day, said owner Andy Baldwin. He had ordered triple the amount of food he normally does for his three sandwich shops in the area.

"I think the hurricane did a good job of keeping everybody away," Baldwin said. "Hopefully, business will pick up."

But some businesses ? such as The Hub, which bills itself as America's best dive bar ? already were picking up. Daytime bartender Jeannie Robinson said several delegates and protesters stopped in on Monday, doubling the usual weekday business.

"I have my Mitt Romney T-shirt," Robinson said. "Even though I'm a Democrat."

2.) Get the biggest cabin you can?afford so you don?t feel closed in and confined. You will feel like you are in a luxurious hotel rather than on a ship!

3.) Let your travel consultant chose the cabin?s location. You want to be in the middle of the ship, on a lower level, to prevent as much motion as possible.

4.) Stay hydrated! Drink lots of water and eat lightly (if you can resist all the goodies!) for the first day or two.

Are Your Pets Allowed On the Furniture? [POLL]

Last night, when I got home, I walked inside and turned on the lights. Everything seemed as normal as always until I looked over at my couch and saw the way one of my dogs was sitting. Check out the picture I took.

As funny as I found this when I first saw her laying like that, I wasn?t mad. I allow my dogs to lay on the furniture. They?re routinely checked for ticks and fleas and they?re not out all day killing rodents so I have no problem with them lounging around in the same places I do.

I began to think about other friends and family who have pets. After a few minutes, I realized that a lot of these people don?t let their dogs or cats on the furniture. Whether it be because of cleanliness, obedience or a combination of both, they prefer to keep their pets off the couch, chair and bed.

I never found a reason to establish that rule with any dog I?ve ever owned. I think it makes the relationship between dog and owner stronger. I could be naive, but I?m confident I?ll keep it the way it is.

Romney Tax Returns Show Strategy for Moving Money

Republican presidential candidate Mitt Romney and his wife, Ann, have used sophisticated estate- planning techniques for more than a decade to minimize taxes and amass at least $100 million for their family outside of their estate.

The couple created trusts as early as 1995, when Romney was building wealth as chief executive officer of Bain Capital LLC. They packed one for their children with investments that stood to appreciate and set up another for charity that provides a tax deduction and income. The candidate?s retirement account, valued at as much as $87.4 million, may benefit his heirs for decades.

?It?s beneficial for your kids and grandkids to push the money downstream,? said David Scott Sloan, chairman of the national private wealth services estate-planning practice at the law firm Holland Knight LLP in Boston. ?The Romneys appear to be doing things that are similar to what other high-net-worth families do.?

Wealthy couples use strategies allowed under the federal tax system such as moving assets to trusts so that the money may be subject to little or no gift and estate taxes, Sloan said. The Romney family trust is worth $100 million, according to the campaign. That money isn?t included in the couple?s personal fortune, which the campaign estimates at as much as $250 million.

Romney?s use of the tax code to minimize levies for his family has drawn scrutiny from Democrats portraying him as an elite person of wealth who is out of touch with many Americans.

Estate Tax

The Romneys would pay higher taxes under the estate-tax proposals of President Barack Obama and would pay less under Romney?s plan. Obama has proposed increasing the estate tax from current levels and curtailing wealth-transfer strategies. The Republican presidential candidate wants to eliminate the estate tax, which currently applies a top rate of 35 percent and a $10.24 million exemption on a married couple?s combined assets.

A repeal of the levy may save the Romneys about $70 million in federal estate taxes after they both die, assuming the couple?s combined taxable estate was $200 million after deductions for items such as administrative expenses and charitable contributions. Compared with today?s rates, Obama?s proposal may cost the Romneys an additional $20 million.

At the core of the Romneys? estate-planning strategy is a family trust. The technique pushes money out of their taxable estate and lets the couple pay the annual taxes on their family?s investment income.

Future Generations

The Romneys frequently spend time with their five adult sons and their families; the candidate and his wife have 18 grandchildren. The couple established the family trust in November 1995 for their children and other heirs, according to the entity?s 2010 tax return, which was made public by the candidate. Romney released an estimated 2011 income-tax return and he has said he will release the final documents when they are completed.

?Their family trusts are set up to provide for future generations,? Andrea Saul, a spokeswoman for the Romney campaign, said in a statement.

Brad Malt, a partner at the law firm Ropes Gray LLP in Boston, manages Romney?s investments and is the trustee of the family trust. He declined to comment on the family?s estate planning.

The Romneys have transferred assets into the family trust and invested them, amassing a substantial and diversified portfolio of stocks, bonds and alternative investments such as private-equity and hedge funds that generate income. In 2010 alone, the trust realized more than $7 million in long-term capital gains, about $1.5 million in ordinary dividends and $741,407 in U.S. government interest, according to the trust?s tax return.

Gains or Losses

The family trust realized gains or losses from more than 100 securities including those in JPMorgan Chase Co. (JPM), Bank of America Corp. (BAC), Nike Inc. (NKE) and PepsiCo Inc. (PEP) in 2010. It also reported investments in Goldman Sachs Group Inc. (GS) hedge funds as well as interests in hedge funds and private-equity funds managed by Bain Capital, including Sankaty Advisors LLC and an affiliate based in Munsbach, Luxembourg. Because the candidate doesn?t own the trust, he isn?t required to detail its holdings on federal disclosure forms.

Romney co-founded Bain Capital in 1984. When he retired, he entered into an agreement with the Boston-based private equity firm to retain the right to make passive investments in certain Bain Capital entities until 2009, according to financial disclosures filed by his campaign.

Lifetime Limit

The Romneys could have contributed as much as $1.2 million, or $600,000 each, into the family trust in 1995 and invested it in securities or funds they expected to appreciate. They wouldn?t have paid transfer taxes because that was the lifetime limit on tax-free gifts at the time. Even if they contributed more money over time as Congress lifted the gift-tax exemption to the current $10.24 million for a couple, the amount they invested in securities and other assets would have grown at least 10 times to reach $100 million.

Bain Capital has seen its assets under management rise more than 16 times to $65 billion from $4 billion in 1999 when Romney left to run the Winter Olympics in Salt Lake City.

Private-equity funds pool money from so-called limited partners such as pensions, endowments and wealthy families and use that money, usually paired with debt, to buy companies they later sell. The profits are split between the limited partners, who get 80 percent, and the fund?s managers, who receive the remaining 20 percent. That portion, known as the carried interest, is considered a capital gain under current tax law. The top rate on long-term capital gains is now 15 percent.

Romney?s Wealth

Romney has been confronted by persistent questions about his personal finances. His 2010 return shows that he paid an effective tax rate of 13.9 percent on more than $21 million in income, most of it from capital gains and dividends. The candidate said on Aug. 16 that he had never paid less than 13 percent in the past decade and his campaign said the figure he cited represented his share of federal income tax.

Romney?s estimated $250 million in wealth is less than some of his peers who founded private-equity firms at about the same time. Stephen Schwarzman, who created New York-based Blackstone Group LP (BX) in 1985 with Peter G. Peterson, earned $398.3 million in 2006 ? the year before the firm sold shares to the public ? and held 232 million shares of the firm at the end of 2011 worth an estimated $3.14 billion as of Aug. 23. The three founders of Washington-based Carlyle Group LP (CG) each have stakes of about $1.2 billion. The trio earned $134 million each in distributions last year, according to government filings.

?Freeze the Value?

Wealthy investors typically will put their interests in hedge funds or private equity in a trust, or use that entity to invest in such deals directly, said Bobbi Bierhals, an estate- planning partner at the law firm of McDermott Will Emery LLP in Chicago. They usually do it when the fund has a low value because it hasn?t made investments or they haven?t prospered. That way, any future appreciation they anticipate would belong to the trust and be free of estate taxes or additional gift taxes.

Families of such wealth usually consider whether to invest with their personal money or through entities such as a trust or retirement account depending on how big a ?home run? they think the venture may be and its time frame, Bierhals said.

?You want to freeze the value,? she said. ?Put it into a pot where when you get that pop from a sale or IPO, all that additional value goes to the kids.?

?They Are Private?

In some cases, the Internal Revenue Service has challenged valuations of non-publicly traded property transferred to trusts, which can result in significant penalties and back taxes, Bierhals said. Obama?s most recent budget plan included a proposal to require consistency in valuations and restrict discounts taken when taxpayers make gifts.

It isn?t possible to determine from tax documents released by Romney?s campaign the terms of the family trust, assets gifted to it and their initial value, said Jonathan Mintz, chief operating officer at WealthCounsel LLC, a group supporting estate and business planning attorneys.

?That?s one of the real benefits of certain types of trusts and why families use them,? Mintz said. ?They are private.?

Trusts generally serve people with at least $15 million in assets because of the costs involved in setting them up, Bierhals of McDermott Will Emery said. Other political families have set up trusts to benefit future generations, including the Kennedys, according to financial disclosures.

Grantor Trusts

The Romneys structured their family trust as a so-called grantor trust so they pay the tax bill on income generated inside the entity. That?s a strategy wealthy families often use, said Sloan of Holland Knight. The assets inside the trust aren?t depleted by money paid to the government and have more potential to grow outside of the estate.

The payments aren?t considered an additional wealth transfer for gift-tax purposes, and because the Romneys have paid the levies over the years, current distributions to the children aren?t subject to income tax, he said.

Transactions like the one used by the Romneys have drawn scrutiny in Obama?s proposals to raise revenue. The president?s 2013 budget plan would reduce many of the benefits of such trusts by coordinating income and transfer tax rules including how taxes are paid and when gift-and-estate levies apply. That?s because current law ?can result in the transfer of significant wealth by the deemed owner without transfer-tax consequences,? according to the Treasury Department?s explanation of the proposals.

?So Valuable?

Romney?s individual retirement account, which he said in a financial statement filed in June is worth between $18.1 million and $87.4 million, may be used to benefit his children, said John Olivieri, a partner in the private clients group at the law firm White Case LLP in New York.

?IRAs make great estate planning tools because heirs can stretch out the required distributions over their life expectancies,? Olivieri said. ?This is so valuable, in fact, that there was a proposal to stop it.?

When beneficiaries inherit an IRA, they are required to take distributions based on IRS tables that use life expectancies. The younger the beneficiary, the less they have to withdraw each year from the account. That can benefit children or grandchildren because assets in the IRA can continue to grow tax-deferred, said William Schmidt, a partner at the law firm Schmidt Federico, P.C. in Boston specializing in estate planning.

Younger Beneficiaries

Romney, 65, must take distributions from his IRA after he turns age 70? and pay ordinary income tax rates on that money he withdraws, Schmidt said. The account is subject to estate taxes, which means it?s not as tax-efficient of a wealth-transfer tool unless donated to charity, he said.

Senate Finance Committee Chairman Max Baucus, a Montana Democrat, proposed in February to require younger beneficiaries who inherit IRAs to pay taxes over five years instead of spreading them over their lifetime, which would raise an estimated $4.6 billion for the Treasury over the next decade. The plan didn?t advance.

The Romneys also set up a charitable remainder unitrust in 1996, according to state financial disclosure documents. The trust, also known as a CRUT, moves money out of an estate by donating it to charity and has two additional benefits for the creator, said John O. McManus, principal at the law firm McManus Associates in New Providence, New Jersey, and New York.

Charitable Trusts

The maker of the trust receives some tax deduction for the value that is expected to go to charity and receives payments from the trust for a set period before the assets transfer, McManus said.

?The Romneys have been extraordinarily generous in their charitable giving,? Saul, the spokeswoman, said. ?They set up a charitable trust to make a gift to charity.?

The Romneys made $7 million in charitable contributions in 2010 and 2011 combined. The couple gave $1.5 million cash in 2010 and $2.6 million cash in 2011 to the Church of Jesus Christ of Latter-day Saints, tax documents show.

Families who consistently pass money on through a variety of legal estate-planning vehicles like the Romneys can minimize taxes for their estate and future generations, Bierhals said.

?You want to combine all of those strategies to get the most runs,? Bierhals said. ?A lot of estate planning is small ball. It?s singles and doubles and every once in a while you hit a home run.?
